CONSENT TO GO ON WALKS OR VISITS/PHOTOGRAPHS TO BE TAKEN/PUT ON SHERE FUN WEB SITE

I hereby give permission for the above named child to go on accompanied visits or walks with the staff of 'Shere Fun'

I understand that every care will be given for each child's safety and well being and that if they travel in any private vehicle, age appropriate seat belts will be worn at all times.

I also give my permission for photographs of the above named child to be shown to others and possibly used in displays or newspapers to promote 'Shere Fun' or placed on Shere Fun website.
